 Not really sure what is going on other than I will be driving in town and all of a sudden the truck will get loud like as if the truck was cold. Any one know wtf is going on.

Hmm... Injectors maybe? IP maybe? Diesel gelling you in this colder weather.
Oil pressure issue perhaps - maybe not enough oil?
Just guessing here. Some of these suggestions are easy to check.

The temp sender in the "T" stat manifold is for the PCM . If there is a loose or poor connection at it or a bad wire in the harness to it if it has high resistance or a intermittent open. When this happens the PCM thinks the engine is cold and goes into cold advance. It could also be a sender on it's way out. Also I can cuss with the best of them but please I don't think expressions like wtf are appropriate on this forum especially in the title.

Are you still geting the p0216 code?
If its geting louder the timing is advancing maybe from a bad temp senser or stepper motor

yes still getting the 216 code how does one check the stepper motor

You can look down at it from front of engine its on the passenger side of the injecter pump its only 1-1/2" tall with two wires plug in the top and the linkage on the bottom its hard to see but its there.
with the engine running the linkage will move up (advanced) and down (retarded) for the actual Timing to stay with the (desired) timing

When the engine is running, actual and desired
timing should mirror one another closely. The stepper motor
plunger should move to chase the desired timing.
You should
be able to use a scan tool with the time set mode that will retard the timing and the stepper motor plunger will go down
or an Idle air control stepper motor driver
with the Chrysler jumper to move the stepper motor.

The p0216 code is set because the actual timing and desired timing are 5degrees or more apart or the timing has not changed like it needs too.;)

i am also getting the code 251 as well as the 216 and im not to sure what you mean by the chrysler jumper

How much miles on the IP ?

Code 251 AND 216 could also mean that your IP is going bad.
COULD BE ! ! ! Your should get a scan tool or to a shop with a sanner and check the timing. If not set right, set it right, clear the codes and wait if the come back. If the come back, i would suppose that the Injection pump IS bad.

I have 130 000 km ( 81 000 ) miles on the pump

Sudden loud noise = going into fail-safe mode.

Does your truck stutter just a tiny bit when this happens? Does your cruise control disengage?

Think PMD. When it burps, timing resets to fail-safe, advance difference exceeds 5*, sets code.

PMD doesn't set codes, it causes other codes to set. That's why it's so hard to find.

If it starts stalling or surging, you know where to look.

Well im not really to sure if it stutters......and the only time this has happening was in town i have not noticed it on the highway yet so not to sure about the cruise. It acts exactly like when the engine is really cold, same noise (loud), same power.

Yep... really loud, no guts, feels like you can't get out of your own way? The first time mine happened, I thought my turbo had quit.

I had no codes, things were fine, and suddenly I had a check engine light and a loud, gutless truck! I shut it off, swore a bunch, looked at everything under the hood... and when I started it up again, it was normal. WTF?

And no persistent codes. Hmm.

About the third or fourth time it happened, the truck stalled, too. Then I started connecting the dots to the other symptoms.

That was about when I started getting timing codes showing up.

My yukon does almost the same thing. It doesn't stall but it is loud. If I run the truck and push the stepper motor linkage down it quiets down, Then hit the throttle a few times and the linkage goes back up. Must be to full advance and stays. Should the stepper motor linkage return to the down position when the truck is shut off or when you go to start the truck?
