I'm interested in this too....and my truck is the same years as yours. Hopefully someone can give us a little insight.

I'm swapping and these are what i'm paying: (4 speed is cheaper a 5-speed nv4500 is over a grand.)
NP465 + NP205 $300 (actually its $600 for that and 4 superswampers from a friend I figure the tires are worth $300)
Disc + plate + Flywheel $275 (Used GM parts)
Throwout Bearing $40
Flywheel Bolts $20
My CJ has linkage and pedals for a mechanical cluch (as opposed to hydraulic) So im all set there.
Other costs to consider:
Pedal assembly $50
Clutch master Cylinder $50
Clutch slave Cylinder $50
Clutch Fork $50
Clutch hydraulic hose $50

I finished the conversion (well mostly) of the Th400 to the sm-465 this weekend, Here is what you need to know:
The recommended flywheel and disc for a 6.5 is weird, i ran into problems finding parts and bolted up a gas flywheel and clutch after swapping the ring gear, this looks like its working just fine.
I used an engine hoist though the floor of the truck to raise the trans. Bolted a chain to the back so that it wasn't balanced and needed to be supported in the front, which it would when I got the pilot busing lined up. I took the bellhousing off and put the input shaft into the disc and adjusted the lift to the right height. Then put the bellhousing back on and installed the trans.
I still have to swap the pedals and run the hydraulic line and make a crossmember to support the rear but the hard part is over. The NP205 went in very easily.
